HEALTH DECLARATION
About the health declaration
The health declaration is a series of questions about your clients’ health history and lifestyle habits that has two main goals:
- Personalize the premium based on your client’s medical history.
- Exclude or include some categories of medical conditions from the coverage
The health declaration must be filled out when the following clients purchase Emergency Medical Care coverage:
- Clients 55 to 59, travelling 18 days or more
- Clients 60 or more, all trip durations
Health history and lifestyle habits
The health declaration includes questions about the health history and lifestyle of your clients. Questions cover the following:
- High blood pressure
- Cholesterol
- Diabetes
- Osteoporosis
- Body mass index
- Recent hospitalization
- Neurologic conditions such as TIA, Alzheimer’s and Parkinson’s
- Use of tobacco
The answers provided by your clients to these questions have no impact on the coverage. The conditions assessed here are subject to the stability requirements presented in the Notice regarding pre-existing conditions. You can find more information about stability requirements here.
Covered or excluded medical conditions
The last questions of the health declaration are used to determine whether certain types of medical conditions will be covered or excluded. Here is the list of conditions assessed in these questions:
- Cardiovascular conditions
- Kidney conditions
- Organ transplant
- Pulmonary conditions
- Gastrointestinal conditions
- Cancer
- Gastrointestinal conditions
Once you submit the health declaration you will see which categories of medical conditions are covered and which ones are excluded.
Notice regarding pre-existing conditions for clients who have completed a health declaration.
This notice applies to all medical conditions other than those assessed in the last questions of the health declaration.
