Submit a claim
After you have contacted us, you can submit a claim. Print and fill out the claim form and upload it with the original invoices and/or receipts through our file dropbox page.

Direct payment for dental care and prescription drugs
When visiting a pharmacy or a participating dentist, simply present your Blue Cross identification card before you pay. The amount reimbursed by your insurance will automatically be deducted from the invoice.
If your pharmacy or dentist does not participate in the direct payment program, you will need to fill out a form for your dental or prescription drug claims.
For a disability insurance claim
To receive payments for disability benefits, please register for direct deposit.
